First full draft

Update March 28:  It's done.  Handed over the ms to the Press, and production is about to start.  Will share updates when I have more from editors on a possible release date.

It's getting there. Still need to do more work.  There are few things more satisfying than printing out the whole ms to do mark ups by hand.
